What do you think went well?
- I think the simulation went well. It was hard for me at first, but I realized that I missed some assessment steps that were important. It was clear that the patient was dehydrated so I took steps to try to get the patient more hydrated. I performed safety measures, assessments and interventions. I asked questions regarding his pain, feelings, and medical history. I checked his allergies and educated him with patient teachings on his condition.

- Rashid Ahmed was alert and oriented to person, place and time when entering the room. After 2 days in the hospital, he was showing signs of dehydration, and his skin was warm and dry. He had had loss of muscle strength, diarrhea for 2-3 days, and weakness. He reported nausea and abdominal pain and reports sleeping well.
EBP/PCC Describe the components of an abbreviated head-to-toe physical assessment for Rashid Ahmed.
- Vital signs were checked as well as skin turgor, bowel sounds, and input and output.
EBP/PCC What assessment for Rashid Ahmed would you perform to evaluate his fluid balance, and what did the findings indicate?
- Rashid Ahmed had nausea and vomiting which could be found in his assessment findings when his blood pressure drops, he had reduced muscle strength in both arms and legs, irregular heartbeat, hyperactive sounds in abdomen, and dry mucous membranes. These indicate that he had hypokalemia, he wasn’t getting the proper nutrition, and he was severely dehydrated.
