ASSESSMENT TASK 3: WORKPLACE PROJECT – SEEK AND SHARE FEEDBACK
TASK SUMMARY:
There are two parts to this task:
- Part A: You are to obtain feedback from two clients in your work placement service.
- Part B: You are to share and seek feedback from your work placement supervisor and peers.
PART A – OBTAIN FEEDBACK FROM CLIENTS
INSTRUCTIONS:
For Part A of this task, you will need to give the attached Client Feedback Forms to two clients (or family members of clients).
These clients should be people whom you have worked with during your work placement.
You are to ask them to fill out the form with honest responses.
If your clients prefer to provide their feedback verbally, you can sit with them and fill out the feedback form yourself.
Make sure your clients sign the feedback form when they hand it back to you (or once you have filled it out, if they are giving their responses verbally).

PART B – SEEK AND SHARE FEEDBACK
INSTRUCTIONS:
The purpose of this task is to meet with your supervisor and two of your peers to share and seek feedback about the work placement experience.
You may do this in one meeting, or you may meet with each person separately if it is difficult to organize a suitable time.
Use the template below to guide your discussion. You must complete Section 1 of the template before you have the meeting.
Give a copy of the template to each participant prior to the meeting so they can see what is to be discussed. You will need to take notes about what is discussed in the meeting as directed in the template.
