Assessment Task 2: Workplace Project – Providing Brief Interventions
TASK SUMMARY
This task requires you to develop a wellness plan for three people living with mental health issues. There are four parts to this task:
Part A: You must select three suitable clients, review their history and discuss their circumstances with your supervisor. You must prepare a summary of each person’s history and circumstances. You are also required to explore the wellness plan development options available to you.
Part B: You must organise a meeting with your three clients. During each meeting, you must explain the purpose and limitations of a wellness plan, develop a shared understanding of their goals, preferences and values, support them to identify any cultural factors which may inform their wellness plan and develop a shared, whole of life understanding of what wellness means for the person. Your assessor will observe one of these meetings.
Part C: You are required to arrange second meetings with each of your clients to develop and finalise their wellness plans. You assessor will observe one of these meetings.
Part D: You must then implement the three wellness plans that you have developed in parts A–C. Work with your clients to implement their plans and use the template provided to answers a series of questions.
Part E: Finally, you must prepare a report on the work you have done in Parts A–D.

WHAT HAPPENS IF I GET SOMETHING WRONG?
If your assessor identifies that you did not correctly or adequately demonstrate the skills and knowledge required of this task, you will be given feedback on areas for improvement.
Your assessor will provide you with guidance as to whether you need to:
- Redo the observation at a suitable time after you have considered their feedback ? Undertake further learning and redo the observation at a later date.
Your assessor will provide details about the date, time and location of reassessment.
STUDENT INSTRUCTIONS FOR TASK 2:
For this task, you must work with three clients to develop and implement a wellness plan.
Use the consent forms provided in the Appendix to obtain permission from the relevant personnel who may be involved in this task.
Part A – Assessing each person’s needs
Discuss the requirements of this task with your workplace supervisor to ensure you select three suitable clients. Review their history and discuss their circumstances with your supervisor and colleagues. Prepare a summary of each person’s history and circumstances.
Explore the wellness plan development options which you have available to you. What information and resources can you access to help each person make informed choices about wellness planning? How would you go about accessing the resources that you require for each? Prepare a summary of your results.
Part B – Gathering information
In consultation with your workplace supervisor, arrange separate, one-on-one meetings with each of your clients.
During each meeting, you must explain the purpose and limitations of a wellness plan, develop a shared understanding of their goals, preferences and values, support them to identify any cultural factors which may inform their wellness plan and develop a shared, whole of life understanding of what wellness means for the person.
You must work collaboratively with each client to identify the wellness strategies which have worked for them in the past and to decide who may need to be consulted regarding their wellness plan. You must ensure that your assessor is available to observe at least one of the meetings.

At the end of each meeting, make notes in the person’s file. Ensure that your file notes are in accordance with your organisation’s policies and procedures. Prepare a summary of your notes (ensuring that you protect the privacy of each person) and include a summary of how you met your organisation’s policy and procedures.
Analyse the outcomes of each meeting and identify, access and review additional information, documents and support/services that will be beneficial to the planning process. Summarise your decisions.
Part C – Developing a wellness plan
In consultation with your workplace supervisor, arrange second meetings with each of your clients.
The purpose of the second meetings should be to develop and finalise the wellness plan. You must ensure that your assessor is available to observe at least one of the meetings.

Document each wellness plan. Submit the completed wellness plans to your assessor making sure to deidentify each plan first.
Prepare a critical analysis of each meeting. What went well? What would you do differently next time?
Part D – Implementing a wellness plan
For this part of the task, you must implement the three wellness plans that you have developed in parts A – C of this assessment. Work with your clients to implement their plans and develop a report for each person which answers the following questions. A report template has been provided at the end of these instructions to assist you.
- How did you promote wellness by maintaining a positive and emotionally safe environment?
- How did you respond to people in distress or crisis?
- Explain the situation and strategies that you used to provide prompt and sensitive support.
- How did the support that you provided follow the instructions in the client’s wellness plan or advanced directives?
- How did you discuss the nature of distress with the clients and implement wellness strategies? Describe the strategies which you implemented.
- Did you need to seek emergency assistance or back up support from a supervisor or your colleagues? What support did you seek?
- How did the support which you provided meet the needs of the client?
Once you have completed your report, ask your workplace supervisor to add their comments and to sign off to confirm that you implemented wellness plans in the manner which you have described.
Part E – Report
Prepare a report on the work you have done in Parts A–D. Your report must address:
- The summary of each person’s history and circumstances (developed in Part A)
- The summary of available wellness plan options and resources (developed in Part A)
- The summary of your file notes which you (prepared in Part B)
- The summary of the additional supports/services and information (prepared in Part B)
- The summary of your file notes (prepared in Part C)
- The critical analysis (prepared in Part C)
- A summary of how you managed the information to ensure that privacy and confidentiality was ensured. Reference your organisation’s policy and procedures.
You can structure your report however you like (you may like to use the bullet points as your headings), but must ensure that it is clear, easy to understand and that you have cited all sources researched and used in
your report. When using sources, they must be used to support your work (that is, you must not copy and paste other people’s work into your assessment and use this as the basis of your work – you must write in your own words).
There is no word limit on your report, but your assessor will be expecting to see that you have covered each section in detail. Ensure your report is clearly written and you have proofread and spell checked your work before submitting it.

Assessment Task 3: Workplace Project – Providing Follow-up Support
TASK SUMMARY
For this task you are required to document the work that you have done to support the development of an advanced directive and to implement a wellness plan.
Templates are provided below for you to fill out and submit. It is highly recommended that you read through each section of the template so you know what you need to address. It may also give you a better idea of which clients you can work with and write about – as some clients may not address all of the criteria in the template.

WHEN AND WHERE WILL THIS TASK BE COMPLETED?
- You will need to complete this task in your workplace. You may fill out the templates provided in your own time or at work (where time allows)
WHAT HAPPENS IF I GET SOMETHING WRONG?
If your assessor determines that you have not completed all parts of the template in sufficient detail or your supervisor has not signed it, they will talk to you about resubmission. You will only have to redo the entries in the template that are identified as having issues or obtain sign-off from your supervisor; however, you will need to resubmit the template. Your assessor will discuss an appropriate timeframe for resubmission with you when you are provided with the outcome for this task.
STUDENT INSTRUCTIONS FOR TASK 3
For this task you are required to document your experiences with one client for whom you have supported to develop an advanced directive.
You may have worked directly with each client or been involved with supporting a senior staff member or supervisor. You may be employed at the service or be on a work placement.
Your client report should document the following:
- Developing advanced care directives
- Reviewing an advanced care directive
- Communication
- Your role in providing support
Each section in the template provides guidance as to what you must discuss. Provide clear and detailed responses to each item.
Once you have completed your report, ask your supervisor to confirm that you supported this client in the manner described by asking them to sign the bottom of the report for each client.
Remember privacy and confidentiality principles when completing your report. Refer to clients using terms such as Client A, Client 1 and so on. Do not include any information in you report that could identify your client (such as location, their workplace, names of places frequented by the client, etc.).
