Assessment Task 4: Reflective Journal – Program Implementation

TASK SUMMARY

In this task you will implement and evaluate the plan that you developed in Assessment Task 3. 

RESOURCES AND EQUIPMENT REQUIRED TO COMPLETE THIS TASK:

Resources

  • Access to a computer, printer, Internet and email software (if required)
  • Access to Microsoft Word (or a similar program)
  • Access to a workplace environment

Template 

  • Reflective Journal template (also provided as a Word document if you would prefer to type your responses).

STUDENT INSTRUCTIONS FOR TASK 4:

A template has been provided for you to fill out, which includes questions and/or prompts to guide you in the responses that are required. You can write on the hard copy template provided or you can type out your responses if you feel you may not have enough space or want to write more. There is no word limit for each entry – just ensure that you provide as much detail as possible so that your assessor is able to determine that you have met the requirements of the task.

Implement the plan which you developed in Assessment Task 3. If you are unable to implement the whole plan, select one section or session. You should also gather, evaluate and incorporate feedback, prepare reports and communicate the outcomes of the program along with recommendations to key stakeholders.

Remember to respect the confidentiality of those you refer to (do not provide names or other personally identifiable information of people discussed in your Reflective Journal).

Notes to students: This unit requires you to show evidence of your ability to complete the tasks outlined in this journal as part of your work in a community services environment. Clients, their families and friends and other staff must always be de-identified. 

Part A – Implementing the plan

Reflect on the implementation of your program plan.

Describe the session/program/intervention. What was the delivery strategy? Include a profile of the audience. How many people attended? Etc.

What went well with the session/program?

What would you do differently next time?

Part B – Evaluating the program

Reflect on the process you used to evaluate the session/program.

Describe how you gathered feedback from stakeholders and summarise their feedback. Did you use the feedback mechanism that you developed in Assessment Task 3?

From your own observations and from the stakeholder feedback, did the session/program meet the goals and objectives which you identified in Assessment Task 2?

Was there value in the session/program (value for money, value from resources committed, value to community/groups/individuals, did it meet a need)?

Part C – Recommendations and reporting

Reflect on the recommendations and reporting processes.

Describe the recommendations for change to the program that you made as a result of the feedback from stakeholders and from your own observations. Did you need to

seek approval for changes? Reference the relevant policy and procedures. 

What were the reporting processes and documentation requirements following delivery of the program? How did you ensure compliance with the organisation’s policy and procedures? Reference the relevant policy and procedures.
